"Manufacturer" means anyone who is engaged in manufacturing, preparing, propagating, compounding, processing, packaging, repackaging, or labeling of a prescription drug. No place of manufacturing, wholesaling or repackaging of drugs or medicines, as defined in ORS 689.005(20), (35), and (36) shall be conducted or operated until it has been registered by the State Board of Pharmacy.
"Wholesale drug outlet" means any person who imports, stores, distributes or sells for resale any drugs including legend drugs and nonprescription drugs.
1. Have submitted a complete application on the form prescribed by the State Board of Pharmacy. If applicable, please submit the application and fee for controlled substances. 2. Submit a check for $400 ($450 if controlled substances applied for concurrently) payable to the Oregon Board of Pharmacy. Checks are acceptable. However, all returned checks will be assessed a fee of $25.00 pursuant to ORS 30.701(5). 3. Application shall specify the location of the manufacturer premises. 4. If Out-of-State Manufacturer, Verification Form from Resident State.
Mail the application form, any additional required documents, and payment to the Oregon Board of Pharmacy at 800 NE Oregon St, Suite 150, Portland, OR 97232
NOTE: Change of ownership or location requires a new application, fee and registration within 15 days.
