Description: Assisted Living Facility means a building, complex or distinct part thereof, consisting of full self-contained individual living units where six or more seniors and person with disabilities may reside. The facility offers and coordinates a range of supportive services available on a 24-hour basis to meet the activities of daily living, health, and social needs of the residents as described in these rules. A program approach is used to promote resident self-direction and participation in decisions that emphasize choice, dignity, privacy, individuality, independence, and home-like surroundings.

Prerequisites: No prerequistes for licensee(s).

Prior to applying for building permit, applicant must submit to DHS a Letter of intent including:
  • city, number of units and max capacity, intended payor sources;

  • market study within 12 months; and

  • pro forma including projected revenues, expenditures and residents days by month for first 12 months and cash flow analysis.

Building plans must be filed with Human Services, Facilities Planning and Safety. Plans must comply with Fire and Life Safety codes and the structural and accessibility requirements of the licensing rules.

The Facility administrator must complete
  • DHS approved 40 hr. training

  • be at least 21

  • HS/GED

  • 2 years professional or management experience in the health or social services

  • 20 hrs continuing education per year

  • Pass Criminal History Check
