Description: A "pharmacy intern" is any person who has:
  • Completed the first professional year and is in good academic standing at an ACPE accredited school or college of pharmacy, or
  • Is a graduate of an ACPE accredited school or college of pharmacy, or
  • Holds a certificate from the Foreign Pharmacy Graduate Equivalency Committee, and has passed the Test of Spoken English (TSE) with a score of not less than 50,
  • And is licensed with the Oregon Board as an intern.

Duration: Valid through the fourth May 31st from issue date.

Prerequisites:

Required Documentation:
  • Have submitted an application on the form prescribed by the State Board of Pharmacy.
    NOTE: The moral character statement on page 2 of the application must be written in its entirety. All applicants are subject to a criminal background check.
  • Submit a check for $30.00 payable to the Oregon Board of Pharmacy. A personal check is acceptable. However, all returned checks will be assessed a fee of $25.00 pursuant to ORS 30.701(5).
  • A copy of your Driver's License.
  • A photograph taken within the last 6 months, no smaller than 2"x2".

Mail the application form, payment, copy of your driver's license, and photograph to the Oregon Board of Pharmacy at 800 NE Oregon St, Suite 425, Portland, OR 97232.
