| Description: |
For workers' compensation claims in Oregon, a naturopathic physician must certify to the director of the Department of Consumer and Business Services, prior to treating a patient, that:
You are an Oregon licensed naturopathic physician
You have reviewed the packet of information
Return and submit the completed Naturopath's Statement of Certification form (Form 440-3651, which is located on the back page of the Naturopaths' Guide to Oregon On-the-Job Injuries)
Once you have submitted your statement of certification form, you should review our List of certified care providers to ensure your information is correct. |
