Description: Issued when a vacancy has occurred at a nursing Home and an Oregon licensed Nursing Home Administrator (NHA) is not readily available. A provisional license is a temporary license that remains in effect for 100 days and may be renewed for an additional 100 days. Provisional licenses are specific to a nursing home, thus a new license is required if the employee secures employment at another facility. A provisional license may be issued to a qualifying employee who is knowledgeable about nursing home operations or to a person seeking endorsement/reciprocity from another state.

A provisional license is specific to the facility and is not portable.

Prerequisites:
  1. Baccalaureate degree or higher from an accredited school of higher education (waived if applicant holds an NHA license issued prior to January 1, 1983).
  2. Experience with and knowledge of facility operations or current licensure in another state.
  3. Is of good moral character and in good physical and mental health.
  4. Criminal history free of felony convictions.
  5. Apply to the Board and remit required fee.
