Description: A "Pharmacy technician" is a person licensed by the State Board of Pharmacy who assists the pharmacist in the practice of pharmacy pursuant to rules of the board. Technicians shall complete initial training as outline by the pharmacist-in-charge which includes on-the-job and related education corresponding to the tasks they are to perform.

Licensure Change:
All current pharmacy technicians are required to become certified no later than October 1, 2008, and gives all new pharmacy technicians until October 1, 2008, one year after initial licensure, or prior to the pharmacy technician's 19th birthday, whichever is later, to obtain certification. Required Documentation: 1. Have submitted an application on the form prescribed by the State Board of Pharmacy.

NOTE:The moral character statement on page 3 of the application must be written in its entirety. All applicants are subject to a criminal background check.

2. Submit a check for $35.00 payable to the Oregon Board of Pharmacy. A personal check is acceptable. However, all returned checks will be assessed a $25.00 returned check fee pursant to ORS 30.701(5).
3. A copy of your Driver's License
4. A passport regulation photograph taken within the last 6 months, no smaller than 2"x2".

Mail the application form, payment, copy of your driver's license, and photograph to the Oregon Board of Pharmacy at 800 NE Oregon St, Suite 150, Portland, OR 97232.
