Description: |
A registered nurse, or nurse practitioner who is an employee of a clinic supported by the Oregon Health Division for purposes of providing public health family planning services, which is licensed by the Board under ORS 689.305 may dispense a drugs or devices from the Board's approved formulary to clients for purpose of birth control, the treatment of amenorrhea, hormone deficiencies, urinary tract infections or sexually transmitted diseases. Such dispensing must be pursuant to the prescription of a person authorized to prescribe a drug or device. |

1. Have submitted a complete application on the form prescribed by the State Board of Pharmacy. 2. Submit a check for $75.00 payable to the Oregon Board of Pharmacy. Checks are acceptable. However, all returned checks will be assessed a fee of $25.00 pursuant to ORS 30.701(5).
