Description: These rules describe the requirements that must be met and maintained for those programs that wish or are required to obtain a License or a Letter of Approval for the purpose of providing treatment services.

Examination Information: Programs are required to submit policies/ procedures as part of the application process for the service elements they wish to provide. These are reviewed by our office, and, if it appears that the services described would be in comliance with the administrative rules, a temporary letter of approval will be issued.
We are required to notify the applicant of the results of their application within 60 days from the date it is received.
Continuing Ed Requirements: Clinical and Medical staff providing services are required to obtain and maintain the appropriate license or certification based on the services provided.
Bonding Requirements: Must meet local codes and zoning ordinances.
Insurance Requirements: Programs are required to obtain and maintain liability coverage for the staff and facility.
Prerequisites: Some programs are required to obtain a license or a Letter of Approval prior to commencing services or advertising themselves as a treatment agency.
