Description:	The Notification Form is used by facilities that are required to notify the Oregon Department of Environmental Quality (DEQ) that they are involved in hazardous waste activities. Facilities that have already notified DEQ may also use this form to update information. You must notify DEQ if you fit into any of the following categories: Hazardous Waste Transporter Large Quantity Hazardous Waste Generator Small Quantity Hazardous Waste Generator Treatment, Storage or Disposal Facility Hazardous Waste Recycling Facility Marketer or Burner of Hazardous Waste Fuel Similar Keywords: materials, hazmat
