Description: "Institutional pharmacy" means hospitals and inpatient care facilities where medications are dispensed to another health care professional for administration to patients served by the hospitals or facilities.
"Retail Drug Outlet" means a place used for the conduct of the retail sale, administering or dispensing or compounding of drugs or chemicals or for the administering or dispensing of prescriptions and licensed by the board as a place wherein the practice of pharmacy may lawfully occur.

Institutional drug outlets which also provide outpatient pharmacy services shall also register as retail drug outlets.

Required Documentation: 1. Have submitted a complete application on the form prescribed by the State Board of Pharmacy and fee ($175). If applicable, please submit the application and fee ($25) for controlled substances.
2. Applications shall be accompanied by a floor plan drawn to scale and shall be approved by the Board prior to opening.
3. Verification form of license/registration in resident state (required for pharmacies located outside of Oregon). Applications for registration of out-of-state pharmacies will not be processed without this verification. To prevent delays in processing your request, submit completed verification form or letter from resident state with your application.
4. Submit a check for the full amount payable to the Oregon Board of Pharmacy. Checks are acceptable. However, all returned checks will be assessed a fee of $25.00 pursuant to ORS 30.701(5).
