information concerning the child and the
immunization provided to the Nevada State Health
Division’s statewide immunization registry: Nevada
WebIZ. Click

As a Nevada
WebIZ user, you must prominently post the
Disclosure Poster where patients can read it.
For further details, please thoroughly read the
Instructions for use of Disclosure Poster & Form,
which includes flow-charts regarding both children
and adults for your reference.
It is stated in the regulations that vaccine lot
details (lot#, expiration date, etc.) are required
to be entered if available. Those offices currently
entering their vaccine lot details in an Electronic
Medical Record system (EMR) will not be required to
enter the information in WebIZ.

Enrollment form and User Account Application below
to get started. Click on “WebIZ – Training” in the
menu on the left to find a scheduled training in
your area or to request an in-house training.
