Supportive Living Program, administered by the Illinois Department of Healthcare and Family Services, is a service-enriched housing option available to Medicaid-eligible individuals that allows elderly, age 65 years and older, or adults, ages 22 – 64 years, with disabilities who would otherwise need nursing facility care. SLFs consist of apartments with kitchenettes and private bathrooms while providing services such as intermittent nursing, personal care, medication oversight, meals, housekeeping, laundry and 24-hour security.

IllinoisBenefits.org, sponsored by the Illinois Department on Aging, provides information about health care and other benefits in Illinois

Illinois Cares Rx (formerly Circuit Breaker Pharmaceutical Assistance and SeniorCare) is administered by the Illinois Department of Healthcare and Family Services and provides "wrap around" benefits with the new Medicare Part D prescription drug program. To enroll, fill out the Form IL-1363. (This and related forms are provided by the Circuit Breaker Program, which enrolls qualifying individuals in the Illinois Cares Rx program.
