NOTE: The Underground Storage Tank (UST) and Heating Oil Tank (HOT) programs are in the process of converting most of the listed forms to be online interactive PDF forms. This allows you to fill in the form online and print off a completed copy. This effort started May 1, 2007 and will be on-going until complete. Forms that have been converted are identified with the phrase �INTERACTIVE PDF�.

To take advantage of this service, please consider the following:

  1. To be able to fill in the form on-line, you will need Adobe Reader version 6.0 or higher loaded on your computer. If you have an older version of Adobe Reader, visit www.adobe.com to download a newer version for free.
  2. Each version of Adobe Reader (i.e. 6.0, 7.0 or 8.0) handles the on-line form slightly differently. Expect to see one or more dialog boxes open with messages as to how your computer and current version of Adobe Reader will handle the on-line form.
  3. You can navigate through the form using the �Tab� key or by moving the cursor from fillable space to fillable space. The �Check Boxes� can be activated by clicking on the box and a checkmark will appear, or by pressing the space bar when using the "Tab" key. If you check a box by mistake, merely click on the box, or press the space bar, a second time to remove the checkmark. The text boxes generally accept letters, numbers, common symbols and combinations of characters. In some cases like the �Date Boxes�, you may get an error message if you don�t enter the date in the format specified in the error message.
  4. You will not be able to save an electronic copy of this file with Adobe Reader. After you fill out the form, you must print a copy to save your work.
  5. As is currently the case with handwritten applications, the application must be mailed in with the applicable fee and any required attachments.
