How to Read Your Three-Year Driver History

Pictured below is a sample 3 year history obtained from the DMV Internet site. Driver histories obtained from the Records Section or a DMV Field Office are slightly different. Click on the graphic for information about an item.

Here is an explanation of the actions which would have occurred, in chronological order, to generate the listings in the sample record of Sam A. Driver III shown above.

On 03-15-2001, Sam was cited by law enforcement for Driving with No Insurance. He was convicted of this offense on 04-24-2001. This resulted in Conviction Listing 1 (violation code 019 - Driving with No Insurance). Nevada law specifies a driver license suspension for this offense. However, suspension and revocation dates are not listed on a 3 year driving history. 

As required by law, Sam surrendered his driver license following his conviction. He received an Identification Card in April, 2001. There is no indication of this on the printout, however, due to the fact that Sam reinstated his license, then was suspended again in 2002. Following this second suspension, he was issued an ID card on 03-19-2002.

On 09-01-2002, Sam met the reinstatement requirements for all of his suspensions and/or revocations in effect at that time. He surrendered his ID card and received a driver license. This resulted in the Issue Date listing for Sam's non-commercial Class C license. 

On 09-15-2002, Sam was cited by law enforcement for speeding. He was convicted of this offense on 10-15-2002, resulting in Conviction Listing 2 (Violation Code 431 - Speeding 21 MPH or More Over the Posted Limit). This conviction resulted in 4 demerit points. Nevada law does not require a license suspension for this offense and no withdrawal action was taken.

On 09-30-2002, Sam met the requirements for a Class M Motorcycle license and received a new license with both Class C and Class M listed.

Record Request Date - The date the printout was produced by DMV.

Physical Exp Date - Physical Expiration Date. Commercial drivers are required to pass a physical examination every two years. The driver must obtain a new exam by the date listed. This will show all zeros if the driver has never filed a Commercial Driver License physical.

Licn Type and Class - Individuals may possess an identification card, a Non-Commercial Driver License or Commercial Driver License. Here are the various classes:

Status - Indicates whether the license or ID is valid, expired, suspended, revoked or cancelled.

Permit - Indicates whether the license is a normal driver license, an instruction permit or an ID card.

Issue Date - The latest date a valid license or card was issued. May be the date of original issue, a renewal, a duplicate, an address change or another action such as adding an endorsement.

Expiration Date - The date the license will expire. Licenses expire on your birthday four years after issuance or, if applicable, on the date of immigration expiration/departure. See Driver License Renewal Options.

Restrictions and Endorsements - Restrictions placed upon driving privileges or endorsements granting the driver extra privileges. Restriction A requires the driver to wear glasses or contact lenses. Endorsement F is a limited commercial licensing exemption which allows firefighters, farm employees or active duty military members to drive large vehicles related to their duties. See Nevada Administrative Code Chapter 483 for a complete list of restrictions.

Conviction Listing - These are listings of moving violations in which the motorist was convicted of the offense by a court of law. DMV is not notified of offenses which are dismissed or reduced to non-moving violations.

Demerit Points - The number of active demerit points. Points are assigned upon conviction of an offense and expire 12 months after the conviction date. An accumulation of 12 or more points within 12 months results in a license suspension. The first number in the violation code (Viol Code) indicates the number of points for that offense. Cite Date and Conv Date - The dates the citation (Cite) was issued by law enforcement and the date the motorist was convicted (Conv) by the court of jurisdiction.

State and Court - The state where the court of jurisdiction in the case is located and the DMV code for Nevada courts. Out-of-state courts are listed as 999. See the Online Code Search to find a specific court.

Viol Code and Off Ty - Violation code and offense type. These DMV codes are not the same as the codes listed on Nevada traffic citations. See the Online Code Search. Offense Type is the code reported to the national Problem Driver Pointer System and available to all 50 states.

CMV Off and Haz Mat - Indicates whether the offense was committed in a commercial motor vehicle (CMV) and whether the vehicle was carrying hazardous materials (Haz Mat).
