The 3 following forms are not required documents if your student management system samplers can be used for permanent records management.

  Arizona Public School Register Books (ADE 41-006) Blue
  High School Attendance Cards (ADE 40-111)
  High School Monthly Attendance Cards (ADE 40-112)
 
Please note the appropriate boxes above and submit no later than July 31.
 
Save (File/Save As...)a copy for your records indicating the number of Register Books you will need.   If you have any questions you can call School Finance at (602) 542-5695.
 
School District orders for all counties, except for Maricopa and Pima, will be mailed to your County School Office during July/August. Districts in Maricopa & Pima Counties will be mailed to school district offices. If you would prefer to pick them up at the Arizona Department of Education, please check the box below and we will call you when they are ready.
