ELDERLY PERSONS AND PERSONS WITH DISABILITIES ABUSE PREVENTION ACT
Version 2005 - Revised Forms

The Office of State Court Administrator has prepared instructions and forms for persons seeking a restraining order under the Elderly Persons and Persons With Disabilities Abuse Prevention Act (ORS 124.005 - 124.040).   Litigants should check with their local courts and facilitators to find out whether the court in their county will accept the use of these forms, and for more detailed information about local rules and procedures.   Once completed, the forms should be filed with the clerk of your local court. 

If your case involves complicated legal issues, you may seriously harm your legal rights if you use these forms without consulting with a lawyer. You are strongly urged to talk to a lawyer. You may find a lawyer by calling the Oregon State Bar's Lawyer Referral Service and Modest Means program at 1.800.452.7636. You may also contact your local legal aid office.
