Preview the Data Dictionary that describes the data elements that recipients will report October 1 to 10. The templates for submitting data will be available as soon as the testing to ensure the templates are accurate and usable is completed.

How to Download a PDF ?Clicking on the PDF icon opens a dialog box in your browser that will ask you where you want to save the file. The speed of downloading will depend on your modem speed, internet traffic and your internet provider. Once you have saved the file, you can read and print documents using Adobe Acrobat software. If you do not have this software, a copy of Acrobat Reader or Exchange is required.

Adobe Reader software is available at no charge in various languages as well as operating systems including: Windows XP and Vista, Macintosh, and UNIX. Documentation for installation and operating requirements of Acrobat is included with the software.

Once the software is installed, view the PDF file by opening it in Acrobat Reader.
