The Board of Examiners for Speech-Language Pathology and Audiology was established in 1973 to license and regulate the performance of speech-language pathologists and audiologists for consumer protection.
Applicants & Licensees
New Fees and New Forms!
With the rule changes effective July 1, 2009, we've changed our forms. Please download a new form if you have one that you are using on a regular basis.

Additionally, it is not negotiable whether you pay the new fee or the previous one. Once the rule is effective, the fees are set by rule and cannot be changed.
All regular Licenses / SLPA certificates expire 1/30/2010
Regardless of when they are issued, all licenses (except conditionals) expire January 30, 2010. The fee is not pro-rated, so if you are considering licensure and can wait until December 15, 2009, the fee you pay then will carry over into the next licensing period. Otherwise, be aware that you will be paying the full licensure fee at application and again by January 30, 2010.
