The Office of Disability Services, a division of Student Affairs and the Counseling and Consultation Center, is committed to providing an equal educational opportunity for all qualified students with disabilities. The Office of Disability Services is responsible for initiating and coordinating services for students with disabilities at Huston-Tillotson University.
In order to establish eligibility for services, the Office of Disability Services may require documentation that both verifies the existence of disability and provides a rationale for reasonable accommodations. Individuals seeking accommodations from ODS are required to provide diagnostic documentation from a licensed clinical professional familiar with the history and functional limitations of the student’s impairments.
All documentation must adequately verify the nature and extent of the disability in accordance with current professional standards, techniques and practices. Also, the documentation must be submitted on the official letterhead of the professional. The report should be typed, dated, signed and include the professional credentials of the evaluator including information regarding licensure or certification.
