Parents' Guide to Childhood Immunizations
The Parents' Guide to Childhood Immunizations is a 68-page booklet introducing parents to the 14 childhood diseases and the 10 vaccines that can protect children from them.
Two ways for you to get a copy:

Order a free booklet from the Centers for Disease Control and Prevention (CDC), National Center for Immunization and Respiratory Diseases (NCIRD). This printed bound version features children's drawings.

Print your own copy from links on this page. The online version of the booklet contains the text only, in pdf format (requires Adobe Acrobat Reader software).
Follow these easy instructions for ordering.
- Scroll the Online Order Form until you reach the section labeled "For Providers and General Public." Order number: (99-9035). Updated order number
- Click the checkbox to select the "Parents Guide to Childhood Immunization, 2008"
- Scroll down to the bottom of the ordering page, fill in the shipping information, and submit the order form online.
