USGS CMG FACS Pre-Activity Responsibilities

Before your field activity, fill out an overview form to get an Activity ID from your FACS Coordinator.

To ensure that you are using the latest version of FACS Logs:

1. View the form in your browser.
2. Do a "File|Save As" to download the form from InfoBank|FACS.

The forms are tab-delimited files that may be used in common applications such as: Word, Excel, BBEdit, SimpleText, Vi.

Fill in as much of the Overview Log and Crew Log information as possible.
