Description: The CMAP’s approach to addressing the breast cancer care continuum is to work within and link medically underserved women to the network of Community Health Centers (CHCs) and county health care systems to provide: (1) breast cancer screening and referrals, (2) community outreach and (3) breast health promotion. The CMAP offers breast cancer screenings (i.e., CBEs and screening mammograms) and referral services (i.e., diagnostic mammograms and follow-up services). The project rotates among 14 CHC sites where women 40+ are linked to a medical home to receive a clinical breast exam prior to the CMAP event. At each of the scheduled CMAP events, screening mammograms are provided. Women served through CMAP must be enrolled in MediCal, Medicare, Family Pact, CDPEWC, private insurance, or a reduced fee-for-service plan and must receive a CBE prior to their scheduled mammogram. Women detected with abnormalities or breast cancer will receive follow-up services at SCVHHS or San Mateo Medical.
