A demonstration program to improve health
care outcomes for people with cancer and/or other
chronic diseases by helping them make their way through
the health care system.
HRSA funded six projects for the two-year demonstration,
beginning in September 2008, to support non-medical
health workers, especially in communities with significant
health disparities and barriers to health services.
The workers, also known as patient navigators, help
people learn about chronic disease, such as cancer,
diabetes, cardiovascular disease, obesity and asthma,
then steer them into screening and treatment as needed.
In addition, navigators assist people in finding and
using community services that will help them beat
chronic disease for longer, healthier lives.
