How can I get a copy of my military records?

Most military records are stored at the National Personnel Records Center in St. Louis, Missouri. Other Records are stored throughout the country at National Archives locations

This is also an area where a congressional office can really help. If you are trying to get records from your military service, or that of a family member, feel free to contact my office. However, due to the privacy act of 1974, I need your signed written permission to obtain records on your behalf. Please print out this Authorization Form, and fax it to my district office at 614-818-0887, or mail it to my district office at:

Be sure to included your name, the veteran's name (if requesting records for a family member) rank, social security number, service number if applicable, unit or units served in while in the military, return address and day time phone number.

Also, please download and fully fill out an SF 180 from the National Personnel Records Center web site and include it with your authorization form.

Please be aware, that only a veteran, or his or her nearest living relative if the veteran is deceased is permitted to order copies of personal records.
