An application, evidence of workers' compensation coverage, authorization from Corporation Division to conduct business in Oregon. Plan of Operation which demonstrates how the worker leasing company will meet the safety and health requirements of ORS Chapter 654, the Oregon Safe Employment Act, Principal Place of Business in Oregon. This is the designated Oregon location where copies of all records are to be maintained as required by OAR 436-050-0450. The name and title of the person(s) in Oregon able to respond to inquiries regarding leasing arrangements and client contracts.
