Divorce/Domestic Partner Separation

As an active LANS employee, if you get divorced or separate from your domestic partner, you must follow these steps:

Step 1	Notify the Lab's Benefits Office at (505) 667-1806. The Benefits staff will assist you with with processing the following items: Health Insurance Enrollment/De-enrollment form (pdf) to remove ex-spouse from health and welfare plans. You must supply a divorce decree, or other suitable proof, that your divorce is final. A spouse must be covered on your current health and welfare benefits through the end of the month in which the divorce is final. Designation of Beneficiary for Retirement plans. Log into the Fidelity website to change your designated beneficiary. LANL is required to offer COBRA to the ex-spouse to continue his/her medical, dental, and/or vision insurance. Provide the Lab's Benefits Office with your ex-spouse's current address so the proper paperwork can be sent directly to him/her. This address should be provided when the enrollment form is completed.
