Disability Process

If your physician determines you are or will be disabled, follow these steps for filing your claim:

Step 1 Call the Lab's Disability Coordinator at 667-1203 to schedule a disability interview (See topics for discussion with the Disability Coordinator). You will receive a packet of information, including disability forms, during this meeting.
Step 2 Complete the following disability paperwork and submit to Liberty Mutual:
  • Attending Physician's Statement form, which your doctor completes.
  • Disability claim form, which you complete.
Step 3 If and when Liberty Mutual approves your disability claim and you have met your required waiting period, payments will begin.
Step 4 Review the Lab's policy on Family and Medical Leave, AM 327 (pdf). If you qualify for FMLA, you can complete the appropriate FMLA forms. If you have questions or need assistance, contact an FMLA consultant at Occupational Medicine (HSR-2), 505-665-2620.
Step 5 Stay in contact with the Lab's Benefits Representative and Liberty Mutual case manager to ensure you are up to date on your claim status.
