Conceptos clave
- Las bombas y las explosiones pueden causar lesiones con patrones particulares que rara vez se ven fuera de una situación de combate.
- Se anticipa que la mitad de las primeras víctimas buscará atención médica en el lapso de una hora.
- La mayoría de las personas gravemente lesionadas llegan después que las personas con lesiones leves, quienes pasan por alto el triaje de los servicios de urgencias médicas y van directamente a los hospitales más cercanos.
- Las lesiones predominantes consisten en múltiples heridas penetrantes y contusiones.
- Las explosiones en espacios confinados (edificios, vehículos grandes, minas) y aquellas que causan derrumbes estructurales producen niveles más altos de mortalidad y morbilidad.
- Las lesiones primarias que reportan los sobrevivientes tienen predominantemente su origen en las explosiones en espacios confinados.
- Debe examinarse y evaluarse en forma reiterada a los pacientes expuestos a una explosión.
- Todos los atentados con explosivos tienen la capacidad de provocar una contaminación química o radiológica.
- Los procedimientos de triaje y de rescate nunca se deben retrasar debido a que la víctima puede sufrir una contaminación radiactiva; el riesgo de exposición de las personas a cargo de su cuidado es muy pequeño.
- Las precauciones universales protegen de la contaminación radiológica secundaria tanto a los socorristas como al personal hospitalario de una manera eficaz.
- A aquellas personas que presentan lesiones con cortes en la piel o en las que se observa exposición de las membranas mucosas, se les debe administrar la vacuna contra la hepatitis B (dentro de los 7 días) y la vacuna antitetánica de acuerdo a su edad (si no está al día).
Lesiones por onda expansiva
- Lesiones primarias: producidas por una fuerza de presurización excesiva (onda explosiva) que impacta la superficie del cuerpo.
- Lesión de la membrana timpánica, lesiones pulmonares y embolización gaseosa, lesión de víscera hueca.
- Lesiones secundarias: producidas por proyectiles (fragmentos de bombas o escombros que salen disparados por el aire).
- Traumatismos penetrantes, lesiones causadas por fragmentos de objetos, traumatismos cerrados.
- Lesiones terciarias: lesiones por el desplazamiento de la víctima provocado por el efecto del viento de la onda expansiva.
- Traumatismos cerrados o penetrantes, fracturas y amputaciones traumáticas.
- Lesiones cuaternarias: todas las demás lesiones producidas por una explosión.
- Lesiones por aplastamiento, quemaduras, asfixia, exposición a sustancias tóxicas, reagudización de enfermedades crónicas.
Lesiones primarias por onda expansiva
- Lesiones pulmonares
- Los signos se observan generalmente al momento de la evaluación inicial, pero pueden demorarse en aparecer hasta 48 horas.
- Se reportan con más frecuencia entre los pacientes con fracturas de cráneo, quemaduras en más del 10% de la superficie corporal y lesiones penetrantes en la cabeza o en el torso
- La forma en que se manifiestan varía desde petequias hasta hemorragias confluentes
- Se debe sospechar la presencia de estas lesiones en toda persona con disnea, tos, hemoptisis o dolor de pecho después de la explosión.
- Radiografía de tórax: patrón en "alas de mariposa".
- Para prevenir la hipoxemia, se administra un alto flujo de O2, mediante mascarilla de oxígeno con reservorio, presión continua positiva de aire (CPAP) o por tubo endotraqueal.
- La administración de líquidos es similar a la utilizada para pacientes con contusión pulmonar; hay que asegurar la perfusión tisular, pero evitar la sobrecarga volumétrica.
- La intubación endotraqueal debe hacerse en caso de hemoptisis masiva, ínminente afectación de las vías respiratorias o insuficiencia respiratoria.
- Se debe considerar la intubación bronquial selectiva en caso de fuga significativa de aire o hemoptisis masiva.
- La ventilación con presión positiva puede aumentar el riesgo de una ruptura alveolar o embolia gaseosa.
- Debe hacerse una descompresión inmediata si hay evidencia clínica de neumotórax o hemotórax.
- Se debe considerar la inserción de una sonda pleural como medida profiláctica antes de una anestesia general o el trasporte aéreo.
- La embolia gaseosa puede causar accidentes cerebrovasculares, infarto de miocardio, síndrome abdominal agudo, ceguera, sordera, lesión de la columna vertebral o claudicación.
- Se administra alto flujo de O2; en posición decúbito prono, decúbito lateral izquierdo o posición semi lateral izquierda.
- Se debe considerar transferir al paciente a terapia hiperbárica de O2.
- Lesiones abdominales
- Las estructuras llenas de aire son las más vulnerables (especialmente el colon).
- Estas lesiones pueden presentarse en forma de perforación del intestino, hemorragias (desde petequias pequeñas hasta hematomas grandes), obstrucción de las venas mesentéricas, laceraciones de órganos sólidos y rotura testicular.
- Se debe sospechar su presencia en personas con dolores abdominales, náuseas, vómitos, hematemesis, dolor rectal, tenesmus, dolor testicular e hipovolemia inexplicable.
- Inicialmente los signos clínicos pueden ser leves hasta presentarse el síndrome abdominal agudo o una sepsis.
- Lesión en el oído
- La membrana timpánica es el área más frecuentemente afectada por las lesiones primarias por onda expansiva.
- Los signos de las lesiones en el oído son generalmente evidentes (pérdida auditiva, zumbido de oídos, otalgia, vértigo, sangrado por el conducto auditivo externo, otorrea).
Otras lesiones
- La amputación traumática de cualquier miembro es un indicador de lesiones multiorgánicas
- Las conmociones cerebrales son comunes y fáciles de pasar por alto.
- Considere retardar el cierre primario de las heridas extremadamente contaminadas y determinar si el paciente ha sido vacunado contra el tétanos.
- El síndrome compartimental, la rabdomiólisis y la insuficiencia renal aguda están asociados a los derrumbes estructurales, al rescate tardío, a quemaduras graves y a algunos tipos de intoxicación.
- Se debe considerar la posibilidad de exposición a toxinas inhalables (CO, CN, MetHgb) en casos de explosiones industriales y terroristas.
- Un porcentaje significativo de los sobrevivientes tendrán lesiones oculares graves.
Manejo del paciente
- No hay directrices definitivas en cuanto a la observación, admisión o alta hospitalaria de pacientes.
- La decisión de dar de alta al paciente dependerá de las otras lesiones que presente.
- Ingreso hospitalario de mujeres en el segundo y tercer trimestre de embarazo para hacerles monitorización.
- Debe hacerse un seguimiento cercano a los pacientes con heridas, lesiones en la cabeza, síntomas de estrés o problemas en los ojos y oídos.
- Los pacientes con lesiones en los oídos pueden presentar zumbido de oídos o sordera por lo puede ser necesario entregar la información y las instrucciones por escrito.
