The
Local Registrar may process correction requests for vital record events
that occurred within their municipality. In some cases, these corrections must
be authorized by the State Bureau of Vital Statistics and Registration before
issuing corrected certified copies of the record. Please contact the Local
Registrar in the municipality where the event occurred for further information.
The Local Registrar will require you to provide documentary proof
of the correct information. These requests are approved on a case-by-case
basis, according to statute.
The State Bureau of Vital Statistics and Registration may also process
correction requests for all vital record events occurring in the state.

Legal Name Changes
Following a legal change of name, you may have your birth, marriage
or domestic partnership record corrected by mailing the following to
the address above:
- A certified copy of the original judgment of name change
- The $27 fee in the form of a check or money order made payable
to:
Treasurer, State of New Jersey (this fee includes one certified copy of the
corrected record)
- A letter identifying the vital record to be corrected. Please
include the name as currently reported in the record, the exact date
and place where the event occurred, and in the case of a birth record,
the mother’s
maiden name and father’s name if listed on the vital record.
