Illinois Health Connect is a statewide Primary Care Case Management (PCCM) Program for most persons covered by an HFS Medical Program. People who are enrolled in Illinois Health Connect will have a "medical home" through a Primary Care Provider (PCP).
Illinois Health Connect is based on the American Academy of Pediatricians' initiative to create medical homes to make sure that preventive healthcare is provided in the best setting. A doctor's office or clinic is a better place than an emergency room for getting routine care. Through creating medical homes, the State expects to improve the quality of our client’s healthcare, while at the same time creating cost savings, which will finance the expansion of healthcare coverage through the new All Kids program.
Potential enrollees will choose a single Primary Care Provider who will coordinate and manage their care. Having a single Primary Care Provider will also help people with chronic conditions like asthma, heart disease or diabetes get the treatment and ongoing care they need to minimize the need for hospital care. Illinois Health Connect Goals:
- Improve access to high quality medical care for HFS clients
- Make sure HFS clients have a medical home with a primary care provider
- Assure all clients receive all necessary preventive and primary care, including immunizations and health screenings
- Increase the provider network
- Reduce inappropriate Emergency Department (ED) visits and hospitalizations
Until the referral system is implemented, claims will continue to be processed without a referral. Providers will be notified by AHS three months prior to the full implementation of the referral process.

As part of the statewide PCCM program, the Illinois Client Enrollment Broker will operate in the Managed Care counties. The Managed Care counties include Adams, Brown, Cook, Henry, Jackson, Madison, Mercer, Perry, Pike, Randolph, Rock Island, St. Clair, Scott, Washington and Williamson.
Clients in the Managed Care counties will pick a health plan (Illinois Health Connect, an MCO or an "MCO-like" plan) and a PCP for their "medical home." The Illinois Client Enrollment Broker will conduct all client enrollment and education activities, including mailing choice education and enrollment materials and assisting with the selection of a health plan and PCP in an unbiased manner.
