TO: ALL TRIBAL IV-D DIRECTORS

RE: OCSE-75

Dear Colleague:

This letter is being issued as a reminder for Tribes to submit their OCSE-75 report. The completed form is to be submitted to the Office of Child Support by Tribal IV-D agencies at least 90 days after the end of the Federal Fiscal Year, or by December 29th of each year, starting this year.

The completed form should be sent to the following address:

      Department of Health and Human Services
Administration for Children and Families
Office of Child Support Enforcement
Division of Planning, Research and Evaluation (DPRE)
Fourth Floor West
370 L’Enfant Promenade, S.W.
Washington, D.C. 20447

Please note that the address is the same location where narrative reports were previously sent, but to a different Division. It is important that the form be dated and signed by the Director of the Tribal Child Support Program.
