This Action Transmittal contains instructions for completing the revised OCSE-157 form for states to use to report program status and accomplishments under Title IV D of the Social Security Act (the Act).

AT-04-07, Introduction of Schedule UDC,a financial reporting form for Itemized Undistributed Collections.

Each State is required to submit quarterly reports on Form OCSE-34A, detailing the amount of child support collections that were received and distributed and disbursed during the quarter. The report also includes the amount of child support collections that remain undistributed at the end of the quarter.
