This Action Transmittal contains instructions for completing the revised OCSE-157 form for states to use to report program status and accomplishments under Title IV D of the Social Security Act (the Act).

Each State is required to submit quarterly reports on Form OCSE-34A, detailing the amount of child support collections that were received and distributed and disbursed during the quarter. The report also includes the amount of child support collections that remain undistributed at the end of the quarter.

The introduction and distribution of revised quarterly expenditure and collection reports (Form) OCSE-396A, and Form OCSE-34A, respectively). These forms were approved by OMB for all IV-D expenditures and collections made on or after October 1, 2003.
