Please follow these steps in order for our staff to assist you with the IRS. If you need to reach the IRS directly, you may do so by calling the automated general information line at (800) 829-1040. This number is useful for general information, such as ordering transcripts of your tax returns. Additionally, this link provides answers to frequently asked questions that my office receives about taxes.

Step 1. Download IRS Form 8821: Tax Information Authorization. To assist in the timely completion of an IRS case, please complete IRS Form 8821. My staff must have this form on file before they can begin to work on your case. Please note that this form is available only in PDF form. The freely available Adobe Acrobat reader is required to view and print PDF files. Please remember to sign and date this form.

Under Section 2, titled "Appointee," please use the following:

Step 2. Write a letter. Please tell my staff and me why you need our assistance. It is vital that you clearly describe your problem and all pertinent information regarding your situation (previous contacts with the agency, case numbers, paperwork filed to date, etc.). Please include your telephone number in case we need to contact you.

Step 3. Mail the letter and the Form 8821 to:
