The Ellsworth AFB claims office is no longer responsible for accepting and/or adjudicating household goods claims. The Air Force Claims Service Center (located near Wright-Patterson AFB, Ohio) is now the focal point for this matter and is responsible for accepting and adjudicating all household goods claims. For instructions on how to file a claim with the Air Force Claims Service Center, please call 1-877-754-1212. The Claims Service Center is open 24 hours, 7 days a week to answer your questions and concerns.

Upon delivery of your household goods and noting any damaged or missing items, ensure you submit the DD Form 1840/1840R to the carrier within 70 days after your items were delivered. You may submit this form via mail or fax directly to the carrier listed in block 9 of the form. Or you can submit the form electronically via the Claims Service Center's website listed above. Once that is accomplished, you have nine months to file a claim with the carrier. You also still have the option to file a claim with the Air Force Claims Service Center within two years if you miss the nine month window for filing with the carrier. Please note that submitting the DD Form 1840/1840R does not constitute filing your claim but is only a preliminary step in the claim process.

You may contact the Ellsworth AFB claims office regarding this information or any other claims question at (605) 385-4901 or DSN 675-4901.

Base legal office attorneys cannot discuss criminal law issues with clients nor provide legal advice of any sort over the phone. All personnel eligible for services at the legal office must come in personally to speak with an attorney if they seek legal advice. Most matters for which personnel seek guidance, with the exception of will drafting or will executions, can be dealt with during our walk-in hours on Tuesdays from 0900-1000 and Thursdays from 1200-1300. Personnel who cannot make it to the legal office during these hours, or need a will drafted and/or executed must call (605) 385-2329 or DSN 675-2329 to make an appointment.

The Legal Office provides legal advice and assistance on personal, civil matters to military members, retirees and dependent family members. Some of the topics include, but are not limited to: wills, powers of attorney, notaries, landlord-tenant and lease issues, domestic relations issues, consumer issues and tax assistance.
