REACH US summary

Mission

The mission of the REACH Coalition is to eliminate diabetes-related disparities among African Americans, Asian Americans & Pacific Islanders, and Latinos/Hispanics living in King County. REACH US provides training and technical assistance to clinics, faith-based organizations, community based agencies, and small businesses on its culturally tailored diabetes education and self-management curricula (REACH Model).

Overview

Coalition members collaborate to create a training program to disseminate the REACH model at appropriate community sites and venues within King County. Coalition partners also assist these organizations in implementing the culturally tailored diabetes education and self-management classes with the populations served by these organizations. In addition to training and dissemination of its model, the initiative will work to promote primary prevention and support organizational systems change in order to sustain its efforts.

The following are the three goals of this new initiative:

Goal 1: Foster the implementation of culturally tailored diabetes interventions in multiple community settings.

Goal 2: Promote health via primary prevention activities, including physical activity, nutrition and stress management.

Goal 3: Promote policy change beyond organizational systems for lasting sustainability of efforts to eliminate health disparities and maintain health equity once achieved.
