Uniform Stamp and Yellow Fever Center Requests

 

Many countries require specific travel immunizations for visitors. Providers interested in giving these travel vaccines need to apply for a Uniform Stamp. The Uniform Stamp allows the authorized provider to officially sign the traveler's International Certificate of Vaccination. This Certificate records the vaccinations received and the signature validates this record. The Yellow Fever Center Certification allows the authorized provider(s) to actually purchase and administer the Yellow Fever vaccine.

 

Who can apply:

  • Physician's
  • Physician's Asistant
  • Nurse Practitioner (with prescription-writing privileges)
  • Certified Immunizing Pharmacist

How to apply:

 

1. Complete the application(s) available for download below. If unable to download, please call the Immunization Program for a copy at 971-673-0300.

 

2. Applications must be completed in full, and may be faxed, or mailed to the address below. Any applications with missing or incomplete information will be returned to the applicant.

 

Please note that Certified Immunization Pharmacists applying for either the Uniform Stamp, or to qualify as a Yellow Fever Vaccination Center must include a copy of their certification of completion of an approved pharmacy-based travel course (see application for details).

 

3. Oregon, as well as the CDC, have developed a web-based registry of all authorized yellow fever vaccination clinics. It is very important that this information remain up to date, as travelers will be using the information on these sites to locate vaccine providers in ther area.

 

4. Continued possesion of the stamp, and continued need for its possession will be re-established regularly by completing a questionaire from State Public Health Division. Completion of this questionaire is required.

 

5. If you move/change address or practice, you must notify State Public Health Division immediately.

  • Uniform Stamp Agreement(pdf) 108KB:
    The Uniform Stamp certification alone, is to officially document on a patient's International Certificate of Vaccination that the travel vaccinations were given. The stamp can only be used by the provider it is assigned to and cannot be loaned to another person.
  • Yellow Fever Vaccination Center Agreement (pdf) 144KB: The stamp holder can legally administer the Yellow Fever vaccination after he/she completes an application for Yellow Fever Center certification and receives confirmation paperwork from the Immunization Program. Yellow Fever Center certification can be requested at the same time Uniform Stamp certification is sought, or at a later date if the provider already is a Uniform Stamp holder. The application for Yellow Fever Center certification will be processed once the applicant has been approved for Uniform Stamp certification.

Adverse Event Reporting


Adverse events reported to public providers after vaccination should be submitted to the Immunization Program, Public Health Division, using a Vaccine Adverse Events Reporting System form (VAERS) according to state guidelines. Private providers should report all adverse events directly to VAERS.
