Restaurant Forms If you plan to operate a new or previously licensed restaurant, a Restaurant License Application must be submitted to the Local Public Health Authority in your area. Facilities that are being constructed or remodeled must complete the plan review process locally prior to construction. The license and plan review fees may vary, so contact your Local Public Health Authority for details.
Restaurants must apply for a variance before starting any specialized processing, such as curing or vacuum-packaging foods as described in OAR 333-150-0000 Chapter 3-502.11. Each variance requestor must be able to prove that their method provides equivalent food safety to the Oregon Food Sanitation Rules. Fill out the Variance Application Form, submit it to your local Environmental Health Department, and they will forward your form to the State Foodborne Illness Prevention Program for evaluation. Mobile Unit Applications and Guidelines If you plan to operate a new or previously licensed mobile unit, a Mobile Unit License Application must be submitted to the Local Public Health Authority in your area.
