This page includes links to instructions and applications for 2008 Employer/Union-Only Group Waiver Plans (EGWPs) (see the "Downloads" section below). Also included is a Summary Chart that provides a quick roadmap to the 2008 EGWP application process. Please be advised that the 2008 EGWP application deadline is 5:00 pm Monday, March 12, 2007. Note that the application process differs depending on: - Whether the applicant is a new applicant or an existing contractor;
- Whether the applicant is applying to be a Prescription Drug Plan (PDP) Sponsor, a Medicare Advantage Organization (MAO), or a Cost Plan Sponsor; and
- Whether the applicant is (a) applying to become an Employer/Union Direct Contract EGWP, or (b) is a new or existing PDP Sponsor, MAO or Cost Plan Sponsor applying to offer new employer/union-only group waiver (i.e., "800 series") plans.
The following highlights important requirements of the application process: - All Direct Contract EGWP applicants must be submitted by hard copy;
- All of the "800 series" EGWP applicants must be submitted online through HPMS. See Related Links Inside CMS for the link to the HPMS Registered Users Log In Page;
- All new PDP applicants and certain types of MAO applicants that intend to offer only "800 series" plans without offering individual plans are also required to send an email notification to CMS; and
- Beginning in 2008, all new applicants and existing contractors adding or expanding EGWP service areas will be able to enter their desired service areas directly into HPMS (no application will be required).
