Intent and use of comments Because the Oregon Inpatient Hospital Quality Indicators are based on billing data, the results have limitations. There are many factors that determine health care outcomes that may not relate to hospital performance, but would influence the results presented here. For example, billing data does not provide every detail about a patient’s condition when he or she is admitted to the hospital, nor does it capture everything that occurs during the hospital stay.

Sacred Heart Medical Center The 436 cases for Sacred Heart include people who had only CABG surgery and people who had CABG surgery and valve replacement or repair or another procedure in addition to the CABG during the same operation. Having an additional procedure at the same time is a more serious and complicated operation. It carries higher risk than CABG alone. In 2006, there were 317 patients who had only CABG surgery and 5 of those patients died for a mortality rate of 1.6%. There were 5 deaths in the 119 patients who had CABG plus another procedure during the same operation for a rate of 4.2%.

CoquilleValley Hospital Of the three (3) Congestive Heart Failure mortalities reported, we know with certainty that two (2) of the three (3) were Do Not Resuscitate cases with multiple co- morbid diagnoses. If the numerator and denominator were corrected to exclude these two (2) Do Not Resuscitate cases, there would be one (1) mortality out of thirty-two (32) cases or a mortality rate of 3.1%.

SamaritanPacific Communities Hospital Small rural hospitals are often at a disadvantage when statistics are presented in percentage format. Out of 50 patients with primary diagnoses of Congestive Heart Failure, only 3 passed away at our facility. Upon review, all 3 of those patients were "DNR" patients, which means that they had requested that we not resuscitate them if their hearts failed. We honored those requests. All the hospitals in the Samaritan Health System, including ours, participate in national Quality Initiatives that include Best Practices for the treatment of Congestive Heart Failure. Pneumonia
Wallowa Memorial Hospital Wallowa Memorial Hospital supports the Office for Oregon Health Policy & Research and their use of Quality Indicators to measure and compare hospitals. However, as the OHPR says in their preamble to this report “because the Oregon Inpatient Hospital Quality Indicators are based on billing data, the results have limitations” and “The data analysis method attempts to adjust for some of these factors, but it is not possible to do so perfectly”. According to hospital specific data during this time frame, WMH had 51 patients who had pneumonia as a primary diagnosis, 5 of whom died for a rate of 9.8%. Of these 5 deaths, though, 3 patients were on “Do Not Resuscitate” status and 1 death was not related to pneumonia. Just excluding the DNR cases, the hospital’s rate is 3.9% and more accurately reflects the quality care provided by Wallowa Memorial Hospital.
