Statement of Financial and Other Interests for County Employees and Elected Officials.

Who Files: All County elected officials and certain County employees must file a financial disclosure statement within ten (10) days of appointment and by April 15 of each year under the King County Code of Ethics. County elected officials may file an F-1 (original signature) with the Board of Ethics in lieu of this form.

Statement of Financial and Other Interests for Board and Commission Members. Who Files: Persons appointed by the executive or county council to serve on county boards, commissions, committees and other multimember bodies that are established by ordinance must file a financial disclosure statement within ten (10) days of appointment and by April 15 of each year under the King County Code of Ethics.

King County Consultant Disclosure Form. Who Files: Consultants entering into a contract to provide professional or technical services to the county costing in excess of $2500 shall complete and file a consultant disclosure form. "Consultant" means a person (e.g., individual, partnership, association, corporation, firm, institution or other entity) who by experience, training and education has established a reputation or ability to provide professional or technical services on a discrete, nonrecurring basis over a limited and pre-established term as an independent contractor to the County.
