Family Abuse Prevention Act
(FAPA) Forms - 2008

SB 269 (Or Laws 2007, Chapter 11) provides that the Office of State Court Administrator shall prescribe the content of the instructions and forms used under the Family Abuse Prevention Act (ORS 107.700 - 107.732).

The forms below have been developed by the SB 269 Forms Implementation Workgroup of the Office of State Court Administrator.  Courts and litigants may use these forms.  The forms published below were were finalized in July 2008.

Translations of the final version of the FAPA forms into Spanish and Russian are not currently available but are anticipated to appear on this website in late 2008.  

For more information about applying for, modifying or renewing a FAPA restraining order, you may contact your local court orfacilitator.   Once completed, the forms should be filed with the clerk of your local court.

Revised FAPA Benchguide 2006

The FAPA Benchguide has been revised and updated to include information about legislation passed during the 2005 Oregon Legislative Assembly.  The 2006 revision was a collaborative effort of the Honorable Maureen McKnight and the Honorable Merri Souther Wyatt, with the assistance of the FAPA Benchguide Revision Workgroup of the State Family Law Advisory Committee’s Domestic Violence Subcommittee.   Although copyrighted by the Oregon Judicial Department, the Benchguide may reproduced or copied for personal use or non-profit educational purposes, but not for resale or other for-profit distribution unless permission is obtained from the Education and Training Division, Court Programs and Services Division, Oregon Judicial Department.
