Clinician's Caries Risk Assessment Tool

This assessment tool is intended to help individuals, such as clinicians who work with children on a regular basis assess the oral health of an individual. It is not a standardized assessment tool and is not meant to replace existing clinical evaluation methods. It is intended to help guide clinical decision-making toward preventive care for a child at risk of oral health problems. Additionally, it is intended to provide guidance to a child's parent or caregiver on preventive oral health practice.

The information presented was developed with the advice of CHILD Profile, Washington State Department of Health and the Medical Assistance Administration, Washington State Department of Social and Health Services. Similar messages are contained in the CHILD Profile messages sent to parents of children from birth to six years of age. The messages are also included in Early Periodic Screening, Diagnosis, and Treatment (EPSDT) visits.
