The Factor 2—Need/Extent of the Problem form in the SuperNOFA is a critical part of your application. Take time to complete all requested information. If you have any questions, please contact OHHLHC.

Please note that we have revised Section B of the Factor 2 Form to make it easier to understand. Please download and complete the REVISED Factor 2 Form to the right. Follow the steps outlined here to complete the form correctly.

  1. Download the revised form from the menu to the right. You may still use the Grants.gov form, but you should substitute "County" where "Code" is requested in Block B1.
  2. Section A: Complete Section A according to the instructions on the form.
  3. Section B: HUD needs to know the county where your proposed lead hazard control work will occur. Enter the county location in Block B1. Use one line for each. In cases where your jurisdiction spans two or more counties, provide the name of each county on a separate line.
  4. HUD also needs to know the jurisdiction where your proposed lead hazard control work will occur. Enter the jurisdiction name in Block B2. Use one line for each. Note: a jurisdiction may be a county, city, or other local municipality.
  5. For applicants working in counties, please identify all large cities in the county where work will NOT occur by entering "NOT" before the city name in the space below the name of the county in Block B2.
