Division Updates Form for Requesting Designated Doctor Exams

AUSTIN, TX -- The Texas Department of Insurance, Division of Workers’ Compensation (DWC) has updated the form for requesting a designated doctor examination in order to comply with the statutory changes made to Labor Code §408.0041 under House Bill (HB) 7 and 28 Texas Administrative Code §126.7. 

The new DWC Form 32, Request for Designated Doctor (Rev. 1/07) includes the additional purposes for requesting a designated doctor examination in accordance with Labor Code §409.0041(a).  The form was slightly revised to simplify and streamline the process for requesting a designated doctor. 

System participants may continue to use the non-revised DWC Form 32 (Rev. 10/05) through March 31, 2007.  If requesting an examination for purposes other than maximum medical improvement or impairment rating, the purpose should be listed under “Other” on the non-revised form. The new DWC Form 32 must be used to request a designated doctor examination on or after April 1, 2007.

The Division has centralized designated doctor scheduling from multiple field office locations to the Austin central office.  All completed DWC Form 32s should be mailed to the Division at the following address:
