Laser and Laser Services Registration

For a new Certificate of Laser Registration, the following forms must be completed to register your Laser equipment and Services.

Submit the completed forms along with the appropriate fee.

• BRC Form 301-2 - Application for Registration of Lasers and Laser Services. 
  Only Class IIIb or Class IV Lasers need to be registered.
  Attach Laser Safety Officer(LSO) qualifications.
  If Physician signs as LSO no further qualifications need to be attached.
  
  
• BRC Form 226-1 - Business Information Form
  
  
• To determine the fee for your facility refer to Title 25 Texas Administrative Code §289.204.
  
  
• For Registration and Radiation Safety Requirements for Lasers refer to Title 25 Texas Administrative Code §289.301.
