Below is a list of the criteria for the 2007 Five-Star Awards.

2007 Five-Star Award Criteria for Funeral Homes and Mortuary Services (22k DOC)

  • Timeliness: Complete 91% or better of death registration releases within 10 days of death using TER between January 1, 2007 and September 30, 2007.
  • Training: Attend the 2006 Annual Vital Statistics Conference or one of the 2007 Regional Vital Statistics Conferences.
  • Innovation: Complete 20% or more of all death registrations entirely electronically between January 1, 2007 & September 30, 2007.
  • Collaboration: Recruit at least two physicians to participate in electronic death registration.
  • Recommendation: Have a letter of recommendation (30k .doc) from a local registrar.

2007 Five-Star Criteria for Hospitals, Birthing Centers, and Midwifes (22K DOC)

  • Timeliness: Complete 92% or better of birth registration releases within 5 days of the birth using Texas Electronic Registrar (TER) between January 1, 2007 and September 30, 2007.
  • Training: Attend the 2006 Annual Vital Statistics Conference or one of the 2007 Regional Vital Statistics Conferences.
  • Paternity: Complete better than the state average of potential Acknowledgments of Paternity at your facility from October 1, 2006 through September 30, 2007.
  • Standardization: Use TER-generated AOPs in 92% of births requiring paternity documentation from January 1, 2007 through September 30, 2007.
  • ImmTrac Performance: Complete better than the state average of ImmTrac Enrollment Forms at your facility from November 1, 2006 through September 30, 2007.

2007 Five-Star Criteria for Local Registrars (20K DOC)

  • Timeliness: Average no more than 18.00 days from date of event to filing in the state office for death records received by the state office from January 1, 2007 to September 30, 2007. Local registration offices averaging less than 10.00 days will be awarded an Exemplary Five-Star designation.
  • Improvement: Use Texas Electronic Registrar (TER) for birth and death registration by February 1, 2007.
  • Training: Attend the 2006 Annual Vital Statistics Conference or one of the 2007 Regional Vital Statistics Conferences.
  • Timely Electronic Death Registration: Register 98% fully electronic death records filed locally within one business day of receipt.
  • Managing Paper Death Registration: Using TER-Death, enter Social Security verifications on all paper death records prior to filing with the State.
