FY 2008 Forms

Instructions

Please download and complete Attachment A and B for the service(s) you provide. If you provide both treatment and recovery support you will need to complete separate Attachments A and B for each MOA.

You will also need to complete the FY '08 ATR Board Member Form and ATR Provider Contact Information Form for your organization. Type the organization name on each form.

Completed information should be emailed to: substance.abuse.contracts@dshs.state.tx.us no later than August 30, 2007. Please include the name of your organization and MOA# in the subject or body of the email. If you have questions, please contact your assigned Contract Manager.
