How to Obtain a Case Record

Agency case records of children and adults are only releasable to certain parties and not to the general public. Depending on your relationship to a case, you may or may not be entitled to the information you are seeking. Requests for copies of case records must be received by mail or in person.

  • Step 1. Complete Form 4885, which is used to obtain copies of confidential agency records.
  • Step 2. You will need to know the city where the alleged abuse took place. That will help you determine which of our regional headquarters you need to see it to. For further information on determining the region, refer to our Office Locator page and scroll down to "In which region and district is my county located?
  • Step 3. Send the completed Form 4885 to the appropriate regional headquarters. Put it to the attention of:
