Texas Online - Initial Application for a Massage Therapist or Massage Therapy Instructor Licensure Links and Forms

Do not use the links and forms below if you are already a licensed Massage Therapist or Massage Therapy Instructor and need to renew. This web page is only for new Massage Therapist or Massage Therapy Instructor applicants. If you are unsure about the qualifications required for a license, please see the rules, 25 TAC §141.10 (Massage Therapist) or 25 TAC §141.29 (Massage Therapy Instructor)

Once you submit an online application, your fees and information will be processed within 3 business days and routed electronically to the Massage Therapy Licensing Program .

You should therefore postmark the other required documents and/or forms listed below on or shortly after the date you submit the online application, to avoid a delay in processing time.

The department recommends that you attach a copy of the "Receipt Page" (the final page you are able to print out when you complete the online application) to any documents you submit to our office.


Additional Information Required of Massage Therapist applicants:

1. Submit an official transcript from the Massage Therapy School or the college or university where you completed your massage therapy training program.

2. Submit a copy of your Social Security card.

3. If you have a disability requiring appropriate accommodations in taking the state examination, submit the Request for Disability Accommodation Form (available below). In addition, please attach a statement on letterhead stationary from a professional who is familiar with your disability. This statement must describe the disability for which you require accommodation.


Additional Information Required of Out of State Massage Therapist applicants:

For Applicants that have been licensed/registered for at least two years in another state or jurisdiction that has requirements of practice and massage therapist licensure that substantially conform to those of the State of Texas, as determined by the Massage Therapy Licensing Program .

1. Submit an official transcript from the Massage Therapy School or the college or university where you completed your massage therapy training program.

2. Submit a copy of your Social Security card.

3. If you have a disability requiring appropriate accommodations in taking the state examination, submit the Request for Disability Accommodation Form (available below). In addition, please attach a statement on letterhead stationary from a professional who is familiar with your disability. This statement must describe the disability for which you require accommodation.

4. Submit proof of passing a National Massage Therapy or State Examination.

5. Submit Verification of Licensure form (available below).

For Applicants that have practiced for at least five years in another state or jurisdiction that does not have requirements of practice and massage therapist licensure.

1. If applicable, submit an official transcript from the Massage Therapy School or the college or university where you completed your massage therapy training program.

2. Submit a copy of your Social Security card.

3. If you have a disability requiring appropriate accommodations in taking the state examination, submit the Request for Disability Accommodation Form (available below). In addition, please attach a statement on letterhead stationary from a professional who is familiar with your disability. This statement must describe the disability for which you require accommodation.

4. If applicable, submit proof of passing a National Massage Therapy or State Examination.

5. Submit proof of employment in the practice of massage therapy, for at least 36 hours worked a month, for the last five years. You must submit two or more of the following forms of documentation: employer affidavits; client affidavits; referral affidavits; or W2 form or other IRS documents that verify compensation for Massage Therapy services.


For Massage Therapy Instructor Applicants:

1. Submit proof of High School graduation or GED. A college or university transcript may be submitted in lieu of a High School diploma or GED.

2. Submit proof of completion of a 30-hour course in teaching adult learners. The course may include an instructional certification program, a college level course in teaching adult learners, or a continuing education course in teaching adult learners.
