ALL PROVIDERS:  Please check the Security Administrator information spreadsheet (listed below) to verify we have accurate contact information on file for your organization.  If your information is incorrect, download the Security Administrator or Back-up Security Administrator Application (list below), complete the form and fax it back to the number on the form.  If you have any questions, please call the BHIPS toll-free pagers at 1-866-373-1253 or 1-866-214-0547 and someone will return your call.  It is imperative you notify us of any changes so we will be able to assist you with accessing the new .NET Version of BHIPS.  Thank you for your cooperation.
